Regenerative Medicine: the future of aesthetic treatments

Growth factors, peptides and PRP (platelet rich plasma) have been around awhile in the cosmetic industry. Polynucleotides and exosomes are newer terms trending on social media and the focus of study within the scientific community honed on the future of aesthetics. These new kids on the block are different from their predecessors because they do something fundamentally different: they can reverse cell aging in key cells that are part of our skin – a concept termed bioregenerative aesthetics.

Skin aging is a natural process that is influenced by both intrinsic and extrinsic variables, such as genetic predisposition, hormone changes, UV radiation exposure, environmental toxins (pollution, smoking) and lifestyle (certain diets, poor sleep, high stress). These elements contribute to the development of wrinkles, fine lines, an uneven tone in our skin, and generalized symptoms of aging as well.

The fibroblast is the cell type in our skin that is responsible for the production of collagen, elastin and the extracellular matrix (ECM), which is the milieu surrounding these cells and makes up a large component of our skin. With aging, the amount and quality of collagen and elastin produced by fibroblasts decreases, as does the content of hyaluronic acid and other components in the ECM.

Stem cells are used to promote healing and reverse aging: they have been used for the past decade in dentistry, orthopedics and complex skin wounds. Harvesting the messengers within stem cells is a relatively new bioengineered technology in the field of regenerative science. Likewise, an important population of cells in our blood called platelets, which are in charge of repairing wounds, are also replete with intracellular messengers that promote healing and tissue regeneration and can also be reaped. These messengers are packaged in tiny “envelopes” and are known as exosomes.

Stem cells have the ability to become many different kinds of tissue and exhibit amazing plasticity and regenerative capabilities and have not yet accrued DNA damage as other cells do over the course of years. Platelets, while they have very specific functionality and do not have the ability to become other cells, are constantly being made by our bone marrow and are “young” cells. They have a life-span of 7-10 days. They are the most important cells in our bodies in charge repairing tissue when it is wounded – they also exert many other functions that are vital to our survival.

More formally, exosomes are small cell membrane- bound “packages” filled with biological information such as microRNAs, messenger RNA, lipids, proteins, peptides, vitamins and other vital building blocks, that originate in the cytoplasm (inside the walls of a cell) and are exported out of the cell of origin. They are the means by which cells communicate with their surroundings.

When the information within these exosomes reaches their target cell, they influence gene expression, allowing these older cells to behave like younger, non-damaged cells. This results in the production of proteins and other products such as the ones made by younger cells. It is analogous to “updating the software” in a computer.

These molecules can reverse the aging in their target tissue, allowing for the production of healthier collagen and elastin, decreased inflammation which is a hallmark of aging, increase their metabolic activity and promote better utilization of essential components such as amino acids and vitamins. In the skin this translates into less wrinkles, increases elasticity and firmness, decreases pigmentation in the skin, and helps reverse damage from environmental and external stressors. These are also utilized for hair growth and greatly enhance the health of hair.

Polynucleotides are biocompatible fragments of DNA derived from purified DNA, often sourced from the gonads of fish, such as salmon or trout, known as sodium DNA (or “salmon sperm” on social media). They are used as regenerative agents. These fragments need to reach the dermal layer of our skin via tiny needles or be applied on the skin after laser procedures, RF microneedling or chemical peels. Peptides are short chains of amino acids (usually <50), essentially mini-proteins. They can act as signaling molecules, hormones, or enzyme inhibitors. They are administered via injection, topically or orally, depending on the peptide. They typically bind to specific receptors on the surface of cells to trigger a biological response, like boosting collagen production, muscle growth, or fat metabolism. They act in a more targeted and specific fashion, usually affecting one pathway or receptor type. These molecules are synthesized in labs or derived from natural proteins.

PDGF, or Platelet-Derived Growth Factor, is a naturally occurring protein in the body produced by platelets, which plays a critical role in regulating cell growth and division, especially in connective tissue cells (such as skin) and is involved in wound healing and tissue repair. It’s one of the key growth factors released by platelets when the body responds to injury—sending signals to nearby cells to activate regeneration. PDGF acts as a “first responder” after an injury.

Each zone of the face receives a custom regenerative treatment – it combines all three of the procedures described above and involves 5 treatments over approximately 2 months:

  • Initial treatment: Full face treatment which includes Light Eyes, Bacio Lip Stimulator and Bioactive Complex, followed by application of a Hydrinity HA mask to lock in treatment, increase product penetration and reduce inflammation
